The aim is to lead people out of the criminal justice system and into intensive case management where those who are dealing with behavioral health issues can have their needs met appropriately.

A specialty team that partners closely with many community partner agencies as part of a continuum of services, engaging clients seeking housing support who are either unenrolled from a health plan or seeking enrollment. The CORS Team aims to help clients access housing resources and other necessary services. They are reliable consultants regarding mental health, case management, and other community resources to clients and other local agencies within their service area. Services in this program include Care Coordination, Housing Referral and Support, and Outreach and Engagement.

A voluntary program for current or former foster youth, aged 15 through 22, who would like to receive supportive case management to establish and achieve goals in various areas of their lives.
Eligible families can receive assistance with employment and housing, benefit navigation and application assistance, affordable Internet connectivity, and more.
All case management services are free and voluntary and are offered to biological relative and foster caregivers. Services include case management for families navigating a crisis or the after-effects of trauma, and on-call services.

The GRACE Program provides intensive case management, coordination of services, and ongoing support.
